X-Men Vol 2 #41 is the issue in which the Age of Apocalypse reality is created. It was published February 1995 and was the finale to the Legion Quest story arc.
Summary[]
Beginning in the Prime Universe (Earth-616), David Haller, the mutant known as Legion, had traveled back in time in an attempt to kill Magneto before he even became an enemy of Charles Xavier. Legion had planned on stopping Magneto early on so that life would be better in the future.
Legion didn't realize that some of the X-Men had followed him into the past. Bishop, Psylocke, Iceman, and Storm tried to stop Legion before it was too late, but Legion proved to be too powerful.
While Legion dealt one final blow to Magneto, Xavier, sacrificing himself for his friend, leaped in front of Magneto. Xavier took Legions attack head-on and paid for it with his life.
At the time of Charles Xavier's death, the alternate reality was created. With Xavier dead, he never would have fathered Legion and never would have formed the X-Men. The super-powered battle caught the attention of Apocalypse. As space and time began to crumble and cease to exist, the M'Kraan Crystal warped time and restructured a new reality, a reality known as the Age of Apocalypse (also known as Earth-295). Years later, without Xavier and the X-Men to stop him, as in the Prime Universe, Apocalypse came into power and ruled the world in this alternate Earth, giving it the nickname "Age of Apocalypse".
