David Haller is the future son of Charles Xavier in the Prime Universe (Earth-616). In his reality, Erik Lehnsherr would become a super villain and constantly battle David's father, Xavier. Using his time-warping powers, David travels back in time to the point where Xavier and Erik Lehnsherr had just recently met each other. Planning to kill Erik before he can become evil, David attacks him. Xavier, trying to save his new friend, ended up taking the brunt of David's attack. So Xavier was killed instead of Erik.
With Xavier dead, David would never have been born and never formed the X-Men, which means, if David never existed, he couldn't travel back in time to kill his father. Since the paradox was created, space and time itself began to fall apart.
The M'Kraan Crystal then recreated the reality, which came to be known as the "Age of Apocalypse", or simply "Earth-295".
